The Military Training Network no longer issues continuing medical education certificates.  Instead we will send two separate letters of verification of attendance.  One letter will be for qualified physicians and the other will be for all other corps including physicians in residency.  

The Military Training Network asks for your help to identify each physician who is eligible for CMEs by placing an asterisk (*) next to the individual's name and include all pertinent information such as rank, service, and corps.  When faxing documents, please include on the cover sheet a brief explanation, i.e., Post-Course Report, Nomination Letter, or Annual Report.

Also, if asked to send additional documents, please annotate "additional supporting documents" on the cover sheet, this helps streamline the process for mailing your completed documents to you.

Nomination Packages

Please ensure that your nomination packages (Program Director or Program Administrator) are complete with all required documentation.  The following table is a helpful tool (like we use) to ensure the correct items are submitted.

Nomination for Program Director
  • Endorsement from the Commander or Commanding Officer.  (Request for waiver of any of the information criteria, must be included in this letter of endorsement).
  • Candidate's name, rank, Duty Address, phone, and e-mail address
  • Candidate's Curriculum Vitae Form with detailed experience and recent teaching history in the course for which applying (ACLS, BLS, PALS)
  • A copy of Candidate's Training Site Faculty card for PALS & ACLS, Training Site Faculty or Instructor card for BLS (front and back)
