Application Information & Instructions
Deadline: January 15. Complete applications must be received by January 15 to receive full consideration for Fall matriculation.
You must use Internet Explorer (6.0 or higher).
IMPORTANT NOTES:
Please read admission requirements before completing the application process.
1. Once you begin filling out the application, complete the process in its entirety without time lapse. The system will not allow for long lapses prior to completion of the form. You should prepare a statement of purpose in Word or Word Perfect to paste into the application. The Statement of Purpose must be completed prior to continuing on to the application.
Below is a list of information needed to complete the application:
- Statement of Purpose
- Personal Data, i.e., name, DOB
- Education - name of institution(s), major, degree, date of degree, dates of attendance, GPA
- GRE and/or TOEFL scores and date taken
- Names of individuals writing letters of recommendation
- Employment Experience - list of all relevant research or teaching experience since college
- Accountability-clarify any periods of time since secondary school three months or longer in duration, not accounted for
2 . Prior to submitting your application, you must print the application. Then hit the "submit" button. Once it has been submitted, please mail or FAX the final printed application to:

Supporting Materials: Letters of recommendation (3 required, in sealed envelopes with original signatures), transcripts sent directly from other institutions, GRE scores or waiver request and TOEFL scores (for international students) must be received by January 15 and mailed to:

The School Code for the GRE and TOEFL is 5824, for both the Department and Institution code.
Clinical and Medical Psychology Applicants:
GRE waivers are not granted
- You must complete the Graduate Application Addendum for Clinical Psychology Doctoral Program in addition to the main application form. You can access this form at either of the following two links:

International Students: The application requires a Social Security Number (SSN), which international applicants may not have. If you do not have a SSN, please contact us prior to submitting your application (e-mail janastasi@usuhs.mil or call 301-295-3913 or 800-772-1747). We will assign a temporary identification number for use on the application form.
Due to internal security evaluations, the Graduate Education application is temporarily unavailable. Please check back at a later date.
